Fees vary. California offers an optional state card for $100, or $50 for Medi-Cal patients. Oklahoma charges $100 for two years, or $20 for Medicaid and Medicare patients. Florida charges $75 for a state card. Most states require you to live there. A small number of states, such as Arizona and Nevada, have limited reciprocity or allow out-of-state patients. Rules for visitors change often.
