Short answer: yes, it can happen, but which test you take matters more than how stoned you felt. Why a car beats a room
Volume is the whole story. A car cabin runs about 100 to 120 cubic feet. A living room is ten times that. When someone exhales into a small sealed space, the THC has nowhere to go unless a window is cracked and air actually moves through. Sidestream smoke off the burning end carries more THC than the exhaled puff, and in a closed cabin it just hangs there. Ten minutes of that is a far bigger dose than walking past a smoker on a sidewalk.

Which tests care, and which mostly do not
Oral fluid swabs
This is the one I would worry about most. Swabs pick up the parent THC molecule in saliva, and cutoffs are low, often 1 to 4 ng/mL depending on the lab. If you sat in a hotboxed car an hour before a swab, you can have THC in your mouth that has nothing to do with you smoking. Most collection protocols build in a waiting period and sometimes a rinse, but not all of them do.
Hair
Hair is the other problem child. THC from smoke lands on the outside of the shaft and can be detected there. Labs run washes to strip external contamination, and a good lab will flag an odd pattern, but the risk is not zero. Frequent exposure to heavy smoke accumulates.
Urine
Urine is the common sample for pre-employment and DOT testing. The screening immunoassay cutoff is 50 ng/mL, with confirmation at 15 ng/mL. Getting there from passive smoke alone takes a lot: a sealed space, heavy smoking, a long exposure, probably repeated sessions. The old controlled studies put non-smokers in small unventilated rooms with people chain-smoking and did get some screening positives, but confirmation usually came back at trace levels or negative.
Blood
Blood testing is rare outside legal or medical settings, and passive exposure typically will not show up unless the draw happens right after a heavy session in a sealed car.
The variables that decide it
- How sealed the cabin was, and whether outside air was actually moving through it.
- How many people smoked and how much flower or concentrate went in.
- How close you sat and for how long.
- How soon after exposure you got tested. Hours matter more than days.
- Which sample type the lab collected.
- Whether the lab runs confirmation or reports the screen alone.
If a test is on your calendar
Practical stuff. Do not ride in a hotboxed car, especially a small one with the windows up. If you end up in one anyway, get windows down and airflow moving right away, and do not sit in the back seat next to the person smoking. Water helps your body generally but does little for passive exposure, because the amount involved is already tiny. If you are handed an oral fluid swab, ask about the collection protocol and whether a waiting period applies before the sample is taken.
If you test positive and you think exposure caused it, request the confirmation test and ask for the actual number. A screen at 51 ng/mL and a confirmation at 4 ng/mL tell very different stories, and labs keep the raw data.
