Medical Marijuana for Panic Disorder: Research Guide

Short answer

No cannabis product has FDA approval to treat panic disorder. THC, the main intoxicating compound in cannabis, can trigger panic attacks. CBD has thin research, and no trial has tested it as a panic disorder treatment. Panic disorder has treatments with strong evidence: cognitive behavioral therapy and SSRIs.

What panic disorder is

Panic disorder involves repeated unexpected panic attacks plus worry about the next attack. NIMH puts 12-month prevalence at 2.7% of US adults. Lifetime prevalence is 4.7%. Average age of onset is 20 to 24. The disorder often occurs with depression, agoraphobia, and substance use disorders.

THC and panic

THC binds CB1 receptors and raises heart rate. A fast heart rate and chest tightness are also panic symptoms. That overlap can start a panic attack in a person who is high.

does cannabis make anxiety worse

NIDA reports that cannabis use is linked to higher rates of anxiety disorders. Cause and effect is unclear in most studies. Some people use cannabis to self-treat anxiety, then report worse anxiety.

Dose matters. High-THC products and edible doses of 10 mg or more raise the risk of acute anxiety and panic, based on emergency department and poison center data.

CBD

CBD does not bind CB1 receptors the way THC does. One small study gave 600 mg of CBD to people with social anxiety disorder during a public speaking task. Anxiety dropped. That study did not include panic disorder. Panic disorder trials of CBD do not exist as of 2025.

Withdrawal

Stopping heavy cannabis use can cause anxiety, irritability, insomnia, and appetite loss. Symptoms peak in the first week. Anxiety during withdrawal can look like a panic attack. Cannabis withdrawal is listed in DSM-5-TR.

State programs

Qualifying condition lists are set by each state. Most lists do not name panic disorder. Some states include "anxiety" or "anxiety disorders" as a category. Certification rules, possession limits, and purchase records differ by state.

• CBT with panic-focused protocols, 8 to 12 sessions
• SSRIs: sertraline, paroxetine, fluoxetine, escitalopram
• SNRI venlafaxine
• Benzodiazepines for short-term use

FDA-approved cannabinoids exist for other conditions. Epidiolex treats seizures. Dronabinol and nabilone treat chemotherapy nausea. None are approved for panic disorder.
