A few wrote the law so tightly that only a handful of dispensaries can operate, which is why patients in some states drive two hours to fill a order.

What almost every state has in common
- A doctor's certification or written recommendation, in person or by telehealth depending on the state
- Registration with a state agency, plus an application or annual fee
- Purchase and possession caps, usually measured as a 30-day supply or a set gram count
- Dispensary-only sales, with delivery allowed in some states and banned in others
- Zero protection for carrying product across state lines
Qualifying conditions
Cancer, chronic pain, epilepsy, multiple sclerosis, PTSD, Crohn's disease, glaucoma, and terminal illness show up on nearly every list. Beyond that it gets uneven. Some states add opioid use disorder, autism, anxiety, or severe nausea. A few let a physician recommend cannabis for any condition they believe it helps, which sounds wide open but in practice depends on finding a doctor willing to sign.
Getting a card, step by step
- Confirm residency. Most states want a driver's license, state ID, or utility bill.
- See a certifying provider who participates in the state program. Bring records that document your diagnosis.
- Submit the application and fee to the state health department or its contractor.
- Wait for approval, then watch the expiration date. Most cards run one year and need a renewal exam.
Limits and where you can use it
Caps vary a lot. One state allows a 60-day supply, another caps you at 2.5 ounces, another measures in milligrams of THC per 30 days. No state lets you smoke in public, and nearly all ban use in a vehicle, on school grounds, or anywhere tobacco smoking is prohibited. Edibles and tinctures usually travel better than flower if you need something discreet, though the container rules still apply.
Does your card work in another state?
Sometimes. A handful of states accept out-of-state medical cards, often with a registration step or a fee, and a few allow patients to buy but not to carry extras home. Most offer no reciprocity at all. Even between two legal states, moving product across the line is a federal matter, not a state one, so leave it behind.
The CBD-only states
A small number of states permit only low-THC or CBD-rich products, usually through a narrow program aimed at seizure disorders. These laws often come with tight THC limits and few dispensaries, which makes them less useful for pain or sleep issues. If your state is one of them, check whether it recognizes anyone else's card before you assume you can buy.
Federal status, briefly
Cannabis stays Schedule I under federal law. Since 2014, Congress has used appropriations language to block the Justice Department from going after state medical programs, but that protection is renewed yearly and doesn't cover recreational sales or people outside a compliant program. It also does nothing for gun purchases, federal employment, or banking.

Health department page, qualifying conditions, possession cap, and whether the state issues a physical card or just a certificate. Then, if I'm traveling, reciprocity. Get those four things right and most of the confusion disappears.
