What research shows
CBD
Most CBD anxiety studies are small. Many test one dose in a lab. Doses in these trials run from 300 mg to 600 mg. One 2011 trial gave 600 mg of CBD to 10 people with social anxiety disorder before a public speaking test. The CBD group reported less anxiety than the placebo group. The trial lasted one day and included few people.

THC
THC binds CB1 receptors in the brain. Effects depend on dose, tolerance, and product. A low dose can reduce tension for some users. A dose above a person's tolerance raises the chance of anxiety, panic, and paranoia. NIDA lists rapid heartbeat, paranoia, and anxiety as short-term THC effects.
NIDA reports that THC in confiscated flower rose from about 4% in 1995 to about 15% in 2018. Concentrates sold in state programs can reach 60% THC or higher. Products sold today do not match the doses used in the older CBD trials.
US law
Cannabis is a Schedule I drug under federal law. That status limits research. The FDA has approved three cannabis-related drugs. Epidiolex (CBD) treats two rare seizure syndromes. Marinol and Cesamet (synthetic THC) treat chemotherapy nausea. None are approved for anxiety.
State medical programs set their own qualifying condition lists. Some list anxiety or post-traumatic stress disorder. Others list severe pain, seizures, and terminal illness. A card from one state does not work in another.
Forms and labels
- Flower: inhaled effects start in minutes and last 1 to 3 hours.
- Oil and tincture: effects start in 15 to 60 minutes. Drops let you measure milligrams.
- Edibles: effects start in 30 to 120 minutes and last 6 hours or more. Slow onset leads to a second dose too soon.
- Vape and concentrate: high THC per hit. Dose is hard to measure.
In states with lab testing, labels list THC and CBD in milligrams per serving. A 10 mg THC edible and a 10 mg THC capsule are not the same. The liver converts edible THC into 11-hydroxy-THC. That form is stronger and lasts longer.
Risks
- Cannabis use disorder: about 3 in 10 people who use cannabis develop it at some point, per NIDA.
- Withdrawal: irritability, insomnia, and lower appetite in the first week after stopping.
- Drug interactions: CBD can raise blood levels of some drugs, including blood thinners and seizure drugs.
- Driving: THC slows reaction time. Do not drive after use.
- Drug tests: full-spectrum CBD oil may contain THC and can trigger a positive test.
- Teen use: cannabis use before age 18 is linked to problems with attention and memory. A direct link to anxiety is not proven.

Evidence for cannabis in anxiety is thin. CBD trials are short and small. THC can help or harm, based on dose. If you use cannabis, start with a low THC dose, read the label, and keep a log of dose and effect. Tell your prescriber about all cannabis use.
