What the clinical data show

The only randomized controlled trial of a cannabis-based medicine in adult ADHD tested nabiximols, an oromucosal spray that contains THC and CBD. The trial enrolled 30 adults. The dose was titrated over 2 weeks to a maximum of 21.6 mg THC and 20 mg CBD per day, with 4 weeks of maintenance. The authors reported improvement in hyperactivity and impulsivity scores. Measures of attention and cognition did not improve. With 30 people, that result is weak evidence. No dose-response study exists. No long-term trial exists. No trial covers adolescents.

Why no dose chart exists

• Approved drugs have dosing labels. Cannabis does not.
• Product labels in the US are often inaccurate. Testing rules vary by state.
• ADHD symptoms vary by person. Age, weight, other drugs, and liver enzymes change response.
• THC tolerance builds. Amount used tends to climb over time.

THC and CBD act in different ways

THC binds CB1 receptors. It causes intoxication, short-term memory loss, and slower reaction time. CBD does not cause intoxication. CBD trials in ADHD are small, and results are mixed. Interactions and risks

Stimulants such as methylphenidate and amphetamines raise heart rate and blood pressure. Cannabis adds to that. Cannabis can also add to drowsiness from some non-stimulant ADHD drugs, including guanfacine and clonidine. Heavy use is linked to cannabis use disorder, anxiety, and in young users, psychosis. Driving after THC use is impaired. Federal law classifies cannabis as a Schedule I drug. State law governs sale.
