What a smartwatch measures

Apple Watch, Garmin, Fitbit and Samsung track pulse with green LEDs and photoplethysmography, or PPG. The sensor reads light changes under the skin. A PPG number differs from an ECG reading by about 3 to 5 beats per minute in lab tests. Motion, cold skin, tattoos and a loose strap push the error higher.

Alerts that matter

Apple Watch sends a high heart rate notification when the resting rate sits at 120 bpm or above for 10 minutes. Most brands use a window between 100 and 120 bpm. The alert logic covers rest, not exercise. A reading during a cannabis high with no movement counts as a resting reading.

Numbers to treat as a signal

• Over 100 bpm at rest: tachycardia.
• Over 120 bpm at rest: many watches fire an alert.
• Chest pain, shortness of breath or fainting: call 911.

Consumer watches record pulse. They do not diagnose a heart attack, a rhythm disorder or a blocked artery. The FDA cleared watch ECG features to show rhythm strips, not to rule out a cardiac event.

Tracking practice

Log the product, the dose and the time. Note the rate at 15, 30 and 60 minutes. Compare the same product on a day with no other stimulant. Caffeine, nicotine and alcohol all move the number. Share the log with a clinician if the rate stays above 100 bpm at rest for 2 hours or the alert repeats on separate days.

Limits

PPG fails during motion. A wrist reading during a high with hand movement can miss by 20 bpm or more. Cold hands reduce signal quality. Tighten the strap above the wrist bone and hold still for 30 seconds.
