What the trials show
A 2021 randomized crossover trial enrolled 80 adults with PTSD. Participants used three smoked cannabis preparations and a placebo, each for three weeks. The preparations were high in THC, high in CBD, or a mix of both. Scores on the Clinician-Administered PTSD Scale did not differ between cannabis and placebo in a way that reached statistical significance.

What the guidelines recommend
The VA and DoD 2023 guideline recommends trauma-focused psychotherapy, including cognitive processing therapy, prolonged exposure, and eye movement desensitization and reprocessing. It recommends three drugs: sertraline, paroxetine, and venlafaxine. It suggests against cannabis and cannabinoids. Sertraline and paroxetine carry FDA approval for PTSD. Venlafaxine does not, but the guideline lists it as a recommended option.
Legal status
Cannabis is a Schedule I drug under the federal Controlled Substances Act. States run their own medical programs, and PTSD appears as a qualifying condition in many of them. State permission is not FDA approval, and state programs do not establish that cannabis works for PTSD.
Risks
- THC can cause anxiety, paranoia, and racing thoughts. Edibles carry extra risk because absorption is delayed and dose errors are common.
- Cannabis use disorder: NIDA reports about 3 in 10 people who use cannabis develop some degree of use disorder.

- CBD changes how the liver breaks down some drugs, including clobazam and warfarin.
Practical steps if you use cannabis and have PTSD
- Tell your prescriber what you use, how much, and how often. THC and CBD both interact with other drugs.
- Check the product label. FDA tests of CBD products have found items where the CBD content did not match the label.
- Do not stop an SSRI or SNRI on your own. Tapering under a clinician reduces discontinuation symptoms.
- Keep a written log of symptoms and use. The log gives your clinician data that memory does not.

Does CBD treat PTSD?
No health agency has approved CBD for PTSD. Trials of CBD in PTSD are small and short. Epidiolex, the FDA-approved CBD drug, is approved for seizures in three rare epilepsy syndromes.
