What is in a distillate cart
Distillate starts as crude cannabis extract. Producers strip waxes, fats, and plant matter, then run the oil through short-path or wiped-film distillation. Heat separates cannabinoids by boiling point. THC boils near 155 C at low pressure. Terpenes boil between 150 C and 200 C, so most leave the vessel during the run.

The result is a thick oil with one main cannabinoid and few others. Lab reports for THC distillate list total THC between 70% and 95%. CBD distillate sits in the same range. Filled cartridges hold 0.5 g or 1 g of oil. A battery heats a metal coil or ceramic wick, and the user inhales the aerosol.

Distillate vs full-spectrum oil
Full-spectrum and live resin oils keep plant terpenes and minor cannabinoids such as CBG, CBN, and CBC. Distillate does not, unless a producer adds them back. Labels that say "terpene infused" mean terpenes were reintroduced after distillation. The clinical meaning of that difference is not established. Trials have not compared distillate carts with full-spectrum carts for pain outcomes.
What the evidence shows
A 2017 review by the National Academies of Sciences, Engineering, and Medicine named chronic pain in adults as a condition with conclusive evidence for cannabis treatment. That review covered more than 10,000 abstracts. It did not separate inhaled distillate from other routes.
A 2015 meta-analysis in JAMA pooled 79 randomized trials with 6,462 participants. It found moderate-quality evidence for cannabinoids in chronic pain and spasticity. Most trials used nabiximols, a mouth spray, or oral THC. Inhaled products were a minority.
Neither source answers the question of distillate carts as a product category. The route matters for onset and duration. The formulation matters for the mix of compounds. No published trial isolates those two variables for distillate cartridges.
Dose and onset
- Inhaled THC reaches the blood in seconds. Effects start in 1 to 10 minutes.
- Peak effect arrives at 15 to 30 minutes.
- Effects last 1 to 3 hours.
- Oral THC takes 30 to 90 minutes to start and lasts 4 to 8 hours.
A 3-second draw from a 1 g cart at 80% THC can deliver 1 mg to 3 mg of THC. That number varies with draw length, battery voltage, and coil age. Most batteries run 2.5 V to 4.2 V. Higher voltage vaporizes more oil per draw and burns terpenes.
Label checks
- Total THC and total CBD in milligrams, not just percentages.
- Batch test for pesticides, heavy metals, residual solvents, and microbials.
- Vitamin E acetate. Regulated labs screen for it.
- Terpene source. Cannabis-derived or botanical.
- Cart hardware. Ceramic and quartz coils run cooler than nickel or titanium.
Risks
CDC tied vitamin E acetate to the 2019 EVALI outbreak. The agency found the compound in all 29 lung fluid samples it tested from patients. Vitamin E acetate appeared in illicit THC cartridges, not in regulated state-market products.
NIDA reports about 9% of adults who use cannabis develop a use disorder. The rate rises to 17% for people who start in their teens. Daily high-potency THC raises the risk of cannabinoid hyperemesis syndrome, which causes repeated vomiting and abdominal pain.
Distillate carts carry no pain-specific warning. They carry the same risks as other high-THC inhaled products.
Bottom line
Cannabis has evidence for some chronic pain. Distillate carts do not have their own evidence base. A user who wants to test a cart for pain should start with one short draw, wait 15 minutes, and track the result. Talk to a clinician about pain that lasts more than three months.
