The FDA has approved no cannabis or CBD product for use in pregnancy. CBD products are largely unregulated and some contain THC. CBD carries a liver injury risk in the general population.

How THC reaches the fetus

THC is fat soluble. It crosses the placental barrier by diffusion. Researchers have found THC and its metabolites in umbilical cord blood and in meconium, which is the first stool after birth. The fetal brain carries CB1 receptors early in gestation. THC is stored in body fat, so it stays in the body after use stops. It also appears in breast milk.

What the studies show

Human data comes from cohort studies, not randomized trials. Reviews report associations with birth weight under 2,500 grams, birth before 37 weeks, and higher NICU admission rates. Effect sizes are small and results vary between studies. Confounders include tobacco, alcohol, and socioeconomic factors. Smoked cannabis also delivers carbon monoxide and tar, which reduce oxygen delivery to the fetus. Association is not proof of cause, but the pattern appears in several datasets.

Nausea and vomiting

Morning sickness is one stated reason for use. ACOG lists doxylamine plus pyridoxine as first-line treatment. Ginger and acupressure have some support. A clinician can add a prescription antiemetic if those fail. Cannabis is not on the list.

Breastfeeding

THC passes into breast milk. Amounts vary with dose, potency, and how often a person uses. CDC and the American Academy of Pediatrics advise against cannabis while breastfeeding. Long term infant outcomes from exposure through milk are not established.

Product and dose questions

There is no known safe amount in pregnancy. Flower, vapes, edibles, and concentrates all deliver THC. Concentrates hold more THC per dose than flower. Secondhand cannabis smoke contains THC and tar. State-legal labels list THC in milligrams, and that number does not make a product safe during pregnancy. Potency on a label describes strength, not safety.
