Option 2: MET/CBT alone

This is the version most US clinics offer. Two 60-minute MET sessions come first, then 6 to 8 CBT sessions. No urine testing and no vouchers. Abstinence at the end of treatment runs 20% to 30% in published trials. Gains fade after treatment stops unless the person keeps using the skills.

Option 3: Medication, all off-label

No drug has FDA approval for cannabis use disorder. Doctors prescribe a few off-label.

• Topiramate: 200 mg per day or higher. Trials show less use. Side effects include tingling hands, word-finding trouble, and weight loss.
• N-acetylcysteine (NAC): 1,200 mg to 2,400 mg per day. Results are mixed. The clearest signal comes from trials with adolescents.
• Oral THC (dronabinol): eases withdrawal in the first week. It does not raise quit rates in most trials.
• Gabapentin: small trials, mixed results, sometimes used for sleep and anxiety during withdrawal.

What the numbers show

About 3 in 10 people who use cannabis develop cannabis use disorder, per CDC. Withdrawal peaks in the first 7 days and fades over 2 weeks. Sleep problems last longer, up to 30 days in some studies. Heavy use means daily or near-daily use.
